Your initial appointment with OMFS will involve a comprehensive consultation explaining your diagnosis and treatment decisions.
Your initial visit to OMFS will involve completing a new patient registration form, so we ask that you arrive at least ten minutes prior to your scheduled appointment time. We are understanding that at times appointments need to be cancelled or rescheduled so please contact us as soon as you can if this is your situation.
In some cases, your surgery can be performed on the same day as your initial consultation, although this needs to be discussed when booking the appointment (so to allow the appropriate time). During the consultation with your surgeon, a detailed medical history will be taken and treatment plan options discussed.
In most cases the surgery will be booked for a future date either under local anaesthetic (within our consulting rooms) or a general anaesthetic, within a hospital setting.
Please assist us by providing the following information at the time of your consultation:
- Referral document (must be current)
- X-rays if applicable
- Health insurance and Medicare details
- A list of medications you are presently taking
- Next of kin contact information
Please make arrangements to have all documentation available for your visit. If you have private health insurance (hospital or extras cover) please bring the relevant membership cards. This will save time and allow us to help you process any claims. All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, osteoporosis, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
