Why is my full consent necessary?
When you register as a patient of OMFS in Melbourne you provide consent for our specialists and practice staff to access and use your personal information to provide you with comprehensive and tailored medical care. Only the staff who need to see your medical history will have access to it. If we do need to use your information for any other purposes, we will always ask for further consent.
What personal information will I need to give?
Generally, the information our practice staff will ask from you includes:
- Names, date of birth, addresses, contact details
- Medical information which can include medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors
- Medicare number (where available) for identification and claiming purposes
- Healthcare identifiers
- Health fund details
How do you store my personal, confidential information?
We will collect and hold your personal information at our practice in various forms:
- paper records
- electronic records
- visual records (X-rays, CT scans, videos and photos)
- audio recordings
Our practice stores all personal information securely in electronic format that is secured by passwords on an internal computer system.
